Print and fill out this form, and return it no later than May 4, 2007. Your application should include: the completed form, a photo (preferably 8x10), contact information for three references who can speak to your work as an actor, and a 1-page personal statement detailing your reasons for applying.

A $30 non-refundable application fee is required for your application to be processed. Please enclose a check or credit card number with your application form.
