--If you are registering more than one person, you must include EACH additional full name on the back of your form or an attached page.

--If you are registering for a subscriber discount admission, the name and address on your registration form must match that on your 2006 Theatre subscription package.

--If you are registering for a student or senior discount admission, you must include a photocopy of a valid ID showing date of birth for EACH discounted admission.
